The application is now available online. Open the five individual pages, print them, complete them, and return them to your local Rotary Club. If you are unsure about how to contact your local Rotary Club, please check the website of Rotary District 5580 for a club near you. If you are unable to locate a club, contact your school's guidance counselor or email Don Cavalier.
